Researchers analyzed 34 randomized controlled trials involving 1,746 middle-aged and older adults to compare how different resistance training intensities and frequencies affect bone mineral density (BMD) across five skeletal sites. The study used network meta-analysis to rank intervention strategies by their probability of being optimal for each bone location.

For most hip and spine sites—the lumbar spine, femoral neck, trochanter, and Ward's triangle—moderate-intensity resistance training performed three times weekly (3MI) ranked highest in effectiveness probability, with surface under cumulative ranking (SUCRA) scores ranging from 75.2% to 86.0%. When compared directly to control groups, 3MI significantly increased BMD at the femoral neck (standardized mean difference 0.26) and trochanter (0.34). For total body BMD, high-intensity training twice weekly ranked slightly higher. Importantly, the analysis found no statistically significant differences between the top-ranked interventions at several sites, meaning the practical advantage of one approach over another remains unclear. Factors including exercise frequency, intensity, session duration, participant sex, baseline bone health, and age all meaningfully influenced outcomes at different skeletal sites.
