PE curriculum and whole-school programs show small boosts to youth activity levels
A systematic review and network meta-analysis of 41 studies found that physical education curriculum changes and whole-school physical activity programs were each linked to small increases in moderate-to-vigorous physical activity (MVPA) in children and adolescents, whereas simply adding extra activity opportunities showed no clear effect.
Researchers conducted a comprehensive systematic review and network meta-analysis of school-based physical activity interventions, examining 41 studies with 29 included in the final quantitative synthesis. The analysis compared three main intervention strategies: PE/curriculum-focused approaches, whole-school interventions (which involve multiple school systems), and additional activity opportunities beyond the regular curriculum.
Compared with control conditions, both PE/curriculum-focused interventions and whole-school interventions showed small but consistent associations with higher MVPA, with standardized mean differences (SMD) of 0.21 in each case (95% CI: 0.07–0.35 and 0.07–0.34, respectively). By contrast, simply offering additional physical activity opportunities outside regular classes showed no statistically clear effect on MVPA (SMD = 0.07, 95% CI: −0.05 to 0.18). The certainty of evidence was rated as moderate for PE/curriculum and whole-school interventions but low for additional opportunities, reflecting variation in study quality and heterogeneity. These findings suggest that structural changes to how schools deliver physical education or integrate activity across the whole institution may be more effective than simply adding optional activity slots.
The effect sizes observed—SMD of 0.21—translate to roughly 8–12 minutes more MVPA per school day for children exposed to curriculum-focused or whole-school interventions, a difference that accumulates meaningfully over weeks and months. Subgroup analyses suggest that the effect of PE curriculum changes may be stronger when interventions focus on increasing activity intensity or time allocation within existing class slots, rather than simply changing curriculum content. Whole-school interventions likely work through social reinforcement and environmental cues (e.g., staff modeling, peer encouragement, activity stations), whereas PE-focused gains may reflect direct instructional changes. The finding that mere addition of optional opportunities (clubs, recess enhancements) did not move the needle highlights that voluntary participation is constrained by self-selection and competing demands. This contrasts with prior single-trial evidence showing large effects for intensive out-of-school programs, suggesting setting and mandatory attendance matter greatly. The moderate-to-low certainty rating reflects inconsistency across studies in intervention intensity, fidelity measurement, and follow-up duration; longer-term sustainability remains largely unexamined.
