Yoga may outperform combined exercise for pain and fatigue in women with rheumatoid arthritis
A randomized trial of 74 women with rheumatoid arthritis in remission compared 8 weeks of yoga, combined exercise, or usual care on pain, balance, fatigue, and depression. Both active interventions beat control, but yoga showed larger reductions in pain intensity and greater improvements in depression and quality of life at the 20-week follow-up.
Researchers assigned 74 female patients with rheumatoid arthritis (RA) who were in clinical remission to one of three groups: yoga (n=25), combined exercise (n=25), or usual care control (n=24). Participants in the two active groups completed an 8-week supervised programme.
Outcomes were measured at baseline, 8 weeks (post-intervention), and 20 weeks using standardized scales: Visual Analogue Scale for pain, Berg Balance Scale, Timed Up and Go Test, Beck Depression Inventory, Fatigue Severity Scale, and SF-36 quality of life. Both yoga and combined exercise produced significant improvements across all measures compared to control at both time points (p<0.05). Notably, yoga participants showed a median pain reduction of 4.00 points versus 2.00 for combined exercise (p<0.001, η²=0.724), a moderate-to-large effect size. At 20-week follow-up, the yoga group also reported superior outcomes in physical function, balance, fatigue, and depression scores. The researchers propose that yoga's incorporation of breathing and relaxation techniques may explain its edge over conventional exercise. Both interventions appear effective, but this single-centre trial suggests yoga may offer greater psychosocial and pain relief benefits for women with RA in remission.
The pain effect size (η²=0.724) is considered large in clinical research, suggesting yoga's advantage is not trivial. Interestingly, benefits persisted or grew at the 20-week mark despite no intervention between weeks 8 and 20, raising questions about whether participants continued informal practice or whether initial gains create a sustained cycle of reduced inflammation and increased activity tolerance. Subgroup analysis was not reported, so it remains unclear whether findings apply equally to women of all ages or disease durations within remission. The breathing and relaxation emphasis in yoga may dampen the stress-inflammation pathway via vagal tone and cortisol modulation, distinguishing it mechanistically from strengthening-focused combined exercise. One limitation: the trial was single-blinded (assessors blinded, but participants obviously knew their intervention), and the yoga and combined exercise groups differed in attention and contact duration, so some benefit may reflect expectancy or social factors rather than the modality itself.
