The World Health Organization's Global Network of Age-Friendly Cities and Communities (AFCCs) promotes urban environments, policies, and services that support older adults' health and participation. Researchers conducted a systematic review of 17 peer-reviewed studies published between 2017 and 2025 in WHO AFCC signatory cities to examine whether age-friendly features—including accessible transport, walkable streets, and outdoor infrastructure—were linked to directly measured or self-reported health and physical activity outcomes.

Most studies reported positive associations between these urban design features and older residents' physical activity levels or self-rated health. However, the strength of evidence was substantially limited: studies relied heavily on self-reported outcomes rather than objective measurements, used inconsistent methodologies, and varied widely in quality. Few studies employed longitudinal or quasi-experimental designs that could establish stronger causal links. The review found no meta-analysis was possible because outcome measures were too heterogeneous across studies. The authors concluded that while age-friendly city design shows promise for supporting active aging, rigorous evaluation frameworks—including standardised measurement tools, community-based research, and controlled comparison conditions—are needed to inform evidence-based urban health policy in rapidly aging societies.
